is possible on this ext to add a custom image in text area?

To make compatible with the extension alex75/imageintextarea do this:

1. Upload the attachment to /ext/canidev/core/plugins/editor/ (if the folder "editor" inside "plugins" no exists, create it)
2. Go to ACP and clear cache
