Please, Put here the list of extensions installed in the forum
I think that one is giving problems with the editor and cause the debug error.
The same must happen in terms of style, since the extension has been tested in bboots and, in my test forum, it looks correctly.

Seeing the list of extensions, I think the problem may come from the BBCodeOrder + extension, since, like the editor, it assigns icons to the bbcodes.
To be sure I need to know one thing:
In this point, you only disable the ext or you clicked on "deleted data" too?

To add button, I have to switch to prosilver theme. after switching to prosilver I can see now all button in editor.

BBCodeOrder + and cBB Editor used the same column "bbcode_icon" in the bbcodes table so, when you delete one of these extensions, the phpbb installer remove all data related to extension (this column included) and the other extension shows an error.
To solve this, you have two options:
1. Uninstall and reinstall one of the extensions (editor or BBCodeOrder)
2. Do this query through phpmyadmin:
I'll review it, but I do not think you can have these two extensions installed at the same time, since both have the same function of putting icons to the bbcodes
PD: I will check the problem with the style on the ACP
